Labor Support Contract

This Contract is between the birthing parent and partner (hereinafter referred to as "Client"), and Leah White (hereinafter referred to as "Doula") for the purposes of providing labor doula support. After discussion and review, the parties agree as follows:

I. The Prenatal Visit:

  1. Doula will provide one visit prior to the birth (hereinafter referred to as the "Prenatal Visit") at which Client shall discuss a proposed birth plan that reflects Client's needs and desires.

II. The Doula:

  1. Doula works for Client.
  2. Doula has the necessary skills and training so as to enable Doula to perform the services for which the labor doula has been contracted. Doula will support Client's decisions within the limits of Doula's scope of practice.
  3. Doula will articulate and explain all available options and birth plan choices during the Prenatal Visit.
  4. Doula will remain with Client once active labor has begun, or earlier, as requested by Client.
  5. Doula will NOT perform any medical procedures or make decisions regarding medical care for Client and/or baby, as this is outside Doula's scope of practice.
  6. Doula will be on call 24 hours a day starting upon execution of contract by Client.
  7. Doula will be responsible for providing an equally qualified, professional back up doula in the unlikely event Doula cannot attend the birth. Client shall have the right to meet with back up doula if they so choose.
  8. Doula shall strive to create a calm and peaceful environment within the circumstances allowed by the birth location.
  9. Doula will remain with Client until at least one hour after the birth of the baby.
  10. Doula will provide a one-hour follow up postpartum visit within 10 days after the birth of the baby. At this meeting, Doula will answer any and all questions regarding the birth and care of the newborn. Additional postpartum services can be arranged through Doula or Doula can refer client to a qualified and professional Postpartum & Infant Care Doula.

III. Client Responsibilities:

  1. Client agrees to contact Doula at the onset of labor. Client will advise where Doula should report to begin in-person labor support. Doula will report within 2 hours of Client's request.
  2. It is Client's responsibility to contact Doula by TELEPHONE when Client believes labor has begun. This is the only appropriate channel to communicate with Doula regarding labor support; texts, emails, or Facebook messages will not suffice.

IV. Fee for Services:

  1. The fee for labor support is $2000. This payment amount includes one prenatal visit, 1 postpartum follow-up visit (limited to 1 hour), unlimited phone support, and 18 hours of face-to face-labor support. The first half of payment is due at contract signing as a deposit. The remaining balance is due immediately after the birth. Please note that this service is non-refundable.
  2. Additional labor support beyond the 18 contracted face-to-face hours will be billed at the rate of $45 per hour. Client is responsible to pay for any and all parking fees incurred by Doula. These fees will be billed after services are rendered and payment is expected within 14 days of billing.
  3. Doula cannot guarantee the type of labor and birth Client will have. Doula will not refund or negotiate fees based on the circumstances surrounding birth, including medical determinations regarding labor and delivery.
  4. Should Client fail to notify Doula by telephone TWO hours prior to the requested report time and Doula misses the birth, the full fee will be retained by Doula.
  5. The full fee of $2000 will be refunded if Doula misses the birth as a result of reporting to the birth more than two hours after Client's request.
  6. We, the undersigned, have read this Labor Support Contract. We accept and agree to the terms and conditions.
